Agreement to Terms
By downloading, installing, or using ICD-10 Scribe ("the App"), you agree to be bound by these Terms of Service ("Terms"). If you do not agree to these Terms, do not use the App.
Description of Service
ICD-10 Scribe is a medical documentation tool that provides:
- Voice-to-text transcription for clinical notes
- ICD-10 code suggestions based on transcribed content
- Local storage of visit documentation
- Search functionality for ICD-10 codes
License and Usage Rights
Grant of License
We grant you a limited, non-exclusive, non-transferable, revocable license to use ICD-10 Scribe for your personal or professional medical documentation needs.
Restrictions
You may NOT:
- Reverse engineer, decompile, or disassemble the App
- Remove or alter any copyright, trademark, or proprietary notices
- Use the App for any illegal or unauthorized purpose
- Share or transfer your subscription to others
- Use the App in any way that violates HIPAA or other healthcare regulations
Professional Use and Medical Disclaimer
Not a Substitute for Professional Judgment
ICD-10 Scribe is a documentation assistant tool. It is NOT:
- A substitute for professional medical training or judgment
- A diagnostic tool
- A replacement for proper clinical documentation practices
- Certified or approved by any medical board or regulatory agency
User Responsibility
You are solely responsible for:
- Verifying the accuracy of all ICD-10 codes before use
- Ensuring compliance with your organization's policies
- Following all applicable healthcare regulations (HIPAA, etc.)
- Proper clinical documentation practices
- Patient privacy and data security
No Medical Advice
The App does not provide medical advice, diagnosis, or treatment recommendations. All clinical decisions remain your responsibility.
Subscription Terms
Free Trial
- New users receive a 7-day free trial
- Cancel anytime during the trial to avoid charges
- Trial converts to paid subscription unless cancelled
Paid Subscription
- Monthly subscription: $14.99/month
- Annual subscription: $99/year
- Subscriptions auto-renew unless cancelled
- Manage subscriptions through your Apple ID settings
Cancellation and Refunds
- Cancel anytime through App Store settings
- Cancellation takes effect at the end of the current billing period
- Refunds are handled by Apple per their refund policy
- No prorated refunds for partial billing periods
Data and Privacy
Your Data
- All visit data is stored locally on your device
- You own your data
- We do not access, store, or transmit your patient data

Data Security
While we implement security measures:
- You are responsible for device security (passwords, biometrics)
- We are not liable for data loss due to device failure, loss, or theft
- Back up important data through export features
Intellectual Property
Our Property
The App, including all:
- Software code
- ICD-10 database and matching algorithms
- Design elements and user interface
- Logos, trademarks, and branding
are owned by us or our licensors and protected by intellectual property laws.
ICD-10 Codes
ICD-10 codes are maintained by the World Health Organization (WHO) and Centers for Medicare & Medicaid Services (CMS). We provide these codes as reference only.
Disclaimer of Warranties
THE APP IS PROVIDED "AS IS" WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO:
- Accuracy of ICD-10 code suggestions
- Uninterrupted or error-free operation
- Compatibility with all devices
- Fitness for a particular purpose
Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, WE SHALL NOT BE LIABLE FOR:
- Any indirect, incidental, special, or consequential damages
- Loss of data, revenue, or profits
- Medical errors or misdiagnoses
- HIPAA violations resulting from your use
- Any damages exceeding the amount you paid for the subscription
HIPAA Compliance
Your Obligations
As a healthcare provider using this App, you are a HIPAA Covered Entity and must:
- Ensure your use complies with HIPAA regulations
- Implement appropriate safeguards
- Not hold us responsible for your HIPAA compliance
- Train staff on proper use of the App
Our Role
We are a Business Associate providing a tool, but:
- We do not access your patient data
- We do not store data on our servers
- We are not responsible for your HIPAA compliance
- You maintain full control and responsibility
